Geriatrics - Older Adult Services

 

Providence Hospital understands the health concerns and conditions that accompany aging. At Providence, older adults will find a full range of services, from senior wellness centers to primary care centers, specifically designed to promote and maintain their health, independence and quality of life.

Every nurse who joins the Providence team undergoes Age Specific Training. This program educates our nurses on the unique communication skills required for older adults with certain needs, such as those with memory loss. Providence nurses are trained to work with older adults who need extra care, and are able to fully and compassionately communicate with older patients. Nurses are also knowledgeable about the various health conditions that affect older adults and can differentiate what symptoms are attributed to normal aging or are disease related.

Common health conditions that are frequently diagnosed among older patients are Diabetes, Heart Disease, Cancer, Arthritis,and Depression. Each of these conditions can be avoided or managed with the support of experienced team members at Providence who focus on early detection and prevention as well as treatment.

Carroll Manor Nursing and Rehabilitation Center is our skilled nursing center that provides quality short-term and long-term medical, nursing, and rehabilitative care for older adults.

Behavior Health Services offers inpatient and outpatient age-related mental health services that address the changing needs of older adults with mental health concerns, memory problems, and adjustment to the changes in functioning that comes with age.

Center for Geriatric Medicine provides a full range of family and internal medicine services in an outpatient setting for patients 60 years of age and older. Dr. Kathy Brenneman, Dr. David Wilson, Dr. Bindu Joseph and Dr. Deepa Bala oversee the Center for Geriatric Medicine. The center is dedicated to the concept of aging well and maximizing independence. Comprehensive services include:

  • Acute inpatient consultation and care at Providence Hospital and Carroll Manor
  • Coordination of community services
  • Coordination of subspecialty care
  • Preventive health maintenance
  • Pain Management
  • Memory disease evaluation and treatment
  • Assessment and treatment of depression
  • Primary Care
  • Coordination of Palliative Care

Medical House Calls is a program that brings primary healthcare to older or disabled adults who live in the Providence service area and who cannot leave their home in order to get the care that they need.  A multidisciplinary healthcare team provides compassionate care at home that includes:

  • Education and Support for Patients, Family Members, and Caregivers.
  • Preventative Healthcare Services
  • Safety and Environmental Issues Evaluation
  • Scheduled and Urgent-Care Visits
  • Technologically Advanced Diagnostic Tests

Perry Family Health Center and Ft. Lincoln Family Medicine Center also provide a full range of primary care for older adults.

Senior Wellness Centers provide free services that promote the social, physical, and spiritual health of DC seniors.  Services include fitness classes and social activities.

Senior Connection is a hospital-based program which offers information, recreational activities, and special services to adults 60 years of age or older.
