PATIENTS' RIGHTS UNDER HIPAA

Right to a Notice of Privacy Practices

Patients must be given a copy of our Notice of Privacy Practices. We must post it at each service location and on our Web site, and provide it to patients upon request.

Right to Access PHI

Patients may inspect and get a copy of their protected health information (PHI) that we keep, subject to certain limitations. We may require patients to make such requests in writing and may charge a reasonable fee to cover the cost of generating the copy.

Right to Request Amendment to PHI

Patient may ask us to amend or correct their PHI that we keep. We may require patients to make such requests in writing but are not required to grant all requests.

Right to Request Alternative Means of Communicating PHI

Patients may ask us to communicate their PHI by alternative means (such as by fax) or to an alternative location (such as a business address.) We may require patients to make such requests in writing.

Right to Request Restriction on PHI

Patients may ask us to restrict how we use or disclose their PHI. We may require patients to make such request in writing but are not required to grant all restriction requests.

Right to an Accounting

Patients who request it must be given an accounting of certain disclosures of their PHI made by us or by our business associates (that is, disclosures not made for treatment, payment or healthcare operations and where a patient has not specifically authorized release.)

Right to Complain About our Privacy Practices

Patients may make complaints about our privacy practices to our organization and to the Secretary of the U.S. Department of Health and Human Services if they believe we're not complying with HIPAA.
