General Policy
All patient health care information at Providence Hospital is confidential. Therefore, medical record information cannot be released to any person without the authorization of the patient or the patient's legally authorized representative (unless authorized by law).
Request for copies of Medical Records
All requests for the release of medical records must be submitted in writing and must be dated and signed by the patient or the patient's legally authorized representative. In the case of a minor, the parent or guardian must sign the authorization.
Authorization Request Requirement
A Providence Hospital authorization form should be used, although a handwritten authorization is acceptable as long as it contains the following information:
- Patient's name
- Patient's date of birth
- Patient's social security number
- Specific healthcare information requested to be released
- Name of person or organization (and address) to whom the information is to be released
- Reason or purpose for release of the information
- Signature of person authorized to request release of information
Submitting An Authorization
To submit a written authorization for the release of medical information from Providence Hospital, hand deliver or mail the completed authorization to:

If the medical records information request is for continuing patient care, faxing the completed authorization will be accepted. Please fax the authorization to: (202) 269-7931.
E-mail or non-clinical faxed requests or authorizations will not be accepted.
Processing Fees
Fees have been established for copies of patient medical records/medical information. No fee is assessed when the medical information is being sent to another health care provider as part of continuing care.
