Mission Statement of the Diabetes Program
The Diabetes Self-Management Program at Providence Hospital recognizes that diabetes is a chronic disease that is best managed by a knowledgeable and engaged patient working with his/her physician and the program serves to assist individuals with diabetes in translating diabetes knowledge into improved self-care activities.
Program Format
Participation begins with an initial meeting with a course instructor at which time a brief health history is obtained, educational needs are assessed and immediate needs may be addressed. Upon completion of the course, participants will be followed on a yearly basis, or more frequently if needed.
Who Can Attend
Any individual with Type 2 diabetes who has been referred by his/her physician.
You are invited to have a family member attend the program with you.
Is A Referral Necessary
Yes. Most, if not all, insurance companies, including Medicare, require a referral. The referral is also a requirement of the American Diabetes Association's recertification process for diabetes education programs.
Program Hours
Presented twice a month
2 nd Week of the Month - 9 a.m. to 12 noon
4 th Week of the Month - 4 p.m. to 7 p.m.

Covers 12 topics recommended by the American Diabetes Association as part of its national recognition for diabetes education programs, including

- Nutritional Management
- Monitoring The Blood Glucose
- Foot Care
- Managing High and Low Blood Sugars and Other Complications
- Diabetes Medications
