The Center for Geriatric Medicine at Providence Hospital provides the full range of internal medicine services in an outpatient setting for patients 60 years of age and older. We believe that while aging is inevitable, it is not inevitably a negative experience. We are dedicated to the concept of aging well: preventing disease, maintaining wellness and function, treating and controlling chronic illness and maximizing independence to the fullest extent possible for as long as possible. We place strong emphasis on the quality of life, and we respect the dignity of life by serving the whole person with compassion and integrity.
Our team of geriatricians and nurse practitioner provide comprehensive services for seniors, including:
- Primary Care
- Outpatient geriatric assessments
- Preventive healthcare and health maintenance
- Preoperative evaluation
- Coordination of subspecialty care
- Coordination of community services
- Acute inpatient care and inpatient consultation at Providence Hospital
- Primary Care and consultation at Carroll Manor Nursing and Rehabilitation Center
When an initial appointment is scheduled, a "New Patient Packet" is mailed in advance.
This will include a patient questionnaire, medical records release form and a copy of the patient's rights under HIPAA. The completed questionnaire should be returned on the first visit along with valid insurance cards, list of current medications or the bottles and any past medical records or diagnostic studies that may be helpful.
During the first visit, we complete a comprehensive medical and social history, nutritional assessment, complete physical and functional assessment, depression scale and mini-mental examination. We'll then update the patient's medical history and medication list and perform physical exams and diagnostic procedures as necessary on the return visit.
In addition to ensuring close interaction with our medical and support staff, we provide patient education materials, many in large print for the visually impaired.
The Center for Geriatric Medicine is located in Suite 021 on the ground floor of the DePaul Medical Building at Providence Hospital .
