Providence Hospital was founded by the Daughters of Charity of St. Vincent DePaul and chartered by President Abraham Lincoln in 1861. We are the longest continously operating hospital in the Nation's Capital. Our strengths in medicine, surgery and diagnostic services distinguish us as a major DC metropolitan community hospital.   • Carroll Manor Nursing and Rehabilitation Center - a 250-bed, state-of-the-art nursing home;
  • The Perry Family Health Center , a primary health facility in the Northwest #1 area of the city;
  • Seton House, an outpatient substance abuse and psychiatric treatment center.
  • The Congress Heights and Model Cities Senior Wellness Centers include diabetes, depression and hypertension screenings, nutritional assessments, light- weight training or a cardiovascular workout, therapeutic massage, peer counseling, and yoga.
  • Police and Fire Clinic, occupational and preventive medical services to police officers and fire fighters.
  • Fort Lincoln Family Medicine Center, total health care for your family.
  • Model Cities Senior Wellness Center, a wealth of activities for senior citizens.

  • Providence Hospital Wellness Institute in partnership with the DC Office on Aging is in the process of establishing The Hattie Holmes Senior Wellness Center , located in Northwest D.C. This Center will provide services that promote the social, physical, and spiritual health of our DC seniors free of charge.
