Social Services Department
An integral component of the therapeutic services offered at the Carroll Manor Nursing and Rehabilitation Center, is the delivery of psychosocial services to our residents and family members.
The delivery of psychosocial services to our residents and family members is led by the Social Services Department.
The Social Services Department is comprised of professionally licensed BSW and MSW social workers, with experience in the field of gerontology and long-term care programs.
The vast array of psychosocial services provided by the Department include some of the following:
- Advocacy - Advocating for resident rights, including the right to make informed decisions about one's health care and the right to resident dignity and self-determination.
- Service Coordination - Working with residents, families and other health professionals to effectively plan their Nursing Home care. This includes assessing resident needs and recommending referrals to appropriate community resources.
- Discharge Planning - Developing a comprehensive post-discharge plan of care for short-term stay residents, in an effort to return such residents to community living in a safe and appropriate manner.
- Supportive Intervention - Providing therapeutic support and goal-oriented interventions that assist residents and family members with life transitions such as adjusting to the nursing home environment, illness, grief and loss.
